Privacy statement

This privacy statement has been drawn up to inform you of how Logos Praktijk safeguards the privacy of your personal data; I do this in accordance with the GDPR. Logos Praktijk is responsible for the processing of your personal data as set out in this document.

Personal data

Logos Praktijk processes the following categories of personal data:

• name and address details
• contact details such as email address and phone number(s)
• date and place of birth
• gender
• general practitioner
• content of communication (data relating to the contact form, case records, invoices)

Retention period

Logos Praktijk follows the guidelines laid down in art. 6 of the GDPR. Your file is kept in a cloud that fully meets the required quality standards. As the treating counsellor, I am the only one with access to your file. The retention period for files is set by the Dutch government. For client data, Logos Praktijk applies a retention period of 7 years, unless you request earlier destruction of this data. I am bound by a legal duty of confidentiality (professional secrecy) and may not disclose information to third parties without your explicit consent.

Purposes

Logos Praktijk processes this personal data for various purposes, such as: maintaining contact, providing a good service, invoicing, carrying out administrative tasks, collecting payments and taking collection measures, and conducting disputes.

Contact with third parties

Sometimes there is clear added value for the treatment when consultation can take place with other professionals, such as your general practitioner. Contact with third parties about the content of the treatment only takes place with your explicit consent and when consultation is indicated. The information obtained is added to your file, to which I, as the treating counsellor, am the only one with access.

The data in your file may also be used to inform other care providers, for instance when the therapy has been completed or upon referral to another practitioner. Only relevant information is shared, and only with your explicit consent.

Data is shared with third parties in anonymised form for use in peer supervision (intervision). A small part of the data in your file is used for the financial administration, so that I or my administrator can draw up an invoice. Your data is not shared with commercial parties or charities.

Your rights

You have the right to ask Logos Praktijk for access to your file of personal data. After receiving your request, you will receive a complete overview of your personal data within one month. If it reveals inaccuracies, you may request that your data be corrected, supplemented, deleted or restricted.

Security of your personal data

You may ask Logos Praktijk to transfer your personal data. This means that, at your request, I can send the personal data to another person or organisation. If you have the impression that your data is not properly secured, or that it is being misused, please get in touch via the phone number or email address below. If you have a request arising from the rights above, you can submit it via mail@logos-praktijk.nl.

To make sure the request was made by you, Logos Praktijk asks you to enclose a copy of your identity document. To protect your privacy, black out your photo, the machine-readable zone (the strip of numbers at the bottom, MRZ), the passport number and the Dutch citizen service number (BSN) in this copy. I will respond to your request as quickly as possible, within four weeks.

Complaints

If you have a complaint about the processing of your personal data, you can discuss it with Logos Praktijk so that we can reach a solution together. If we cannot resolve it together, I would like to point out your right to file an official complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).

Changes to this privacy statement

This privacy statement was last amended on 8 January 2026. New versions are always published on this website.

Consent requirement

Under the GDPR I am required, before providing psycho-pastoral care, to ask your consent for the processing of your personal data. To demonstrate that my responsibilities and your rights have been discussed, I present this privacy statement for signing.

By agreeing, you indicate that you have taken note of all the rights and obligations explained in this document concerning how Logos Praktijk handles the privacy of your personal data.
